The short answer

The shahadah is the declaration: I bear witness that there is no deity but God, and I bear witness that Muhammad is the messenger of God. Saying it sincerely, with understanding and conviction, is what makes a person Muslim. There is no ceremony, no fee, and no authority whose approval is needed.

A little more context

Witnesses are customary and useful: a mosque can then recognise the new Muslim, and friends can support them. But a person who declares faith alone, or in a hospital bed, is just as Muslim. A bath beforehand is recommended. Everything else, prayer, fasting, and the rest, is learned afterwards, step by step.
